IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Your 2025 health coverage tax form (1095-C) is online at www.foundeverbenefits.com, log in to your account and check the “documents” tab to download it; if you live in CA, NJ, RI, or DC, it will be mailed due to state rules, but you can request it by calling 833-232-7504 for mail or help with online access. This form confirms your employer-offered health coverage and is for your tax records, helping you prove coverage or eligibility for credits if you used the Marketplace.
How to Get Your Form
- Online (Default): Log in to your account at www.foundeverbenefits.com and find it under the ‘documents’ tab.
- By Mail (Mandatory for CA, NJ, RI, DC): It will be mailed if you live in one of these states.
- Request a Mailed Copy: Call Benefitssolver at 833-232-7504 to have it mailed, even if you’re not in a mandated state, or if you have login issues.
What It’s For
- Tax Records: Keep it with your tax documents.
- Proof of Coverage: Shows what health coverage your employer offered.
- Marketplace Credits: Helps you verify eligibility for premium tax credits if you bought insurance through the Health Insurance Marketplace.
Important Notes
- No Need to File: You don’t have to attach Form 1095-C to your tax return, but you need to indicate your coverage status to the IRS.
